Family Support Services Designed Around You
Youth Peer Support (Ages 13–21)
Dedicated youth with lived experience provide mentorship, encouragement, and skill-building support. Programs run up to 12 hours per week to help young people build coping skills, self-advocacy, and confidence.
HCBS (Home & Community-Based Services)
Personalized services delivered at home and in the community to help families develop life skills, manage behaviors, and work toward lasting goals.
Health Home Care Management
Comprehensive care management connecting families to medical, behavioral health, and community resources. We coordinate every part of the care plan to ensure nothing is missed.
Early Intervention Services
For infants and toddlers with developmental delays, early therapy and interventions can make a lifelong difference. We support children in reaching their full potential during these critical early years.
Adult Peer Support (Ages 18+)
We provide core and supplemental evaluations for the Early Intervention Program in New York City.
